Schedule Your Interview
After you have completed filling out your PHS, and you have uploaded all the required paperwork, please click on the Schedule Interview tab and request a meeting with your background investigator.

Waivers / Releases

As part of each background investigation, applicants are required to review and sign the attached waivers and advisements. If you have any questions about any of the documents, please ask your background investigator for an explanation.

Personal History Statement

Your personal history statement is an extensive document that will take some time to complete. Please allow for at least one week to complete the PHS and ensure that all fields are filled in with data or "N/A" if they are not applicable.
